Insurance Program Guidelines

Master policies which cover individual centers for property & general liability, hired & non-owned auto liability, and professional liability will be issued by The General Insurance Company of America (A.M.Best rated “A XV”). Centers may be located in all states other than Hawaii and Louisiana. Insured centers would be added to the policy via coverage certificates. Funding sources and entities to which the centers are obligated by contract to cover as additional insureds are automatically included. Coverage is provided to the Centers for the acts of employed or volunteer Doctors and nurses unless such acts are excluded by the policy (i.e. prescribing drugs or surgical procedures). The master policy would be based in TX.

Basic property coverage applies per location. Liability coverages and limits apply per certificate as if each certificate holder were issued an individual policy. Centers with satellite locations are to be issued with the center and all satellites on one certificate. The basic coverages are as follows:

PROPERTY
  • $50,000 Personal Property of Insured
  • $10,000 Personal Property of Others
  • $5,000 Property in Transit
  • Coverage is provided on the Property Protection Plan form. Other included coverages and limits are shown on the declarations
  • Replacement cost valuation, subject to a $500 deductible
GENERAL LIABILITY
  • $1,000,000 General Aggregate
  • $1,000,000 Products / Completed Operations Aggregate
  • $1,000,000 Personal / Advertising Injury Aggregate
  • $1,000,000 Each Occurrence
  • $200,000 Tenants Property Damage Liability
  • $10,000 Medical Payments
AUTO LIABILITY
  • $1,000,000 Hired and Non-Owned Auto Liability
PROFESSIONAL LIABILITY
  • $1,000,000 Each Occurrence
  • $1,000,000 General Aggregate
ADDITIONAL COVERAGES, LIMITS AND CHARGES
If an account requires higher property limits than those stated above, they can be scheduled on the individual coverage certificate. Higher liability limits are not available. The following are additional coverages that can be purchased:

  • Building: Flat $0.45 per $100 of coverage
  • Contents: Flat $0.50 per $100 of coverage above $50,000
  • Property in Transit: Flat $0.10 per $100 of coverage above $5,000
